HOW TO USE YOUR HSA/FSA 

STEP 1

Contact your HSA or FSA Benefit Specialist or Administrator & find out if you are eligible to use your FSA/HSA card or be reimbursed for Perfectionist Solutions offerings.

Step 2

Ask your physician to complete this Letter of Medical Necessity or to write a prescription indicating Perfectionist Solutions as part of a treatment plan for a specific medical condition, which could include burnout, ADHD, anxiety, exhaustion, depression.


Step 3

Join using your FSA/HSA Card.

Or print and submit your payment receipts for reimbursement to your Insurance Company.
